Some treatment facilities allow you to keep personal information with you during treatment, while others may keep it locked in a safe until you need to use it. Some examples of helpful documents to bring include:

  • Identification
  • Health insurance cards
  • Contact information for your primary care physician
  • Phone numbers for loved ones
